About The Position

Northern Pines Mental Health Center is seeking a Care Coordinator to join their Care Coordination team. This full-time position will primarily work in Brainerd, providing Care Coordination to clients across multiple Northern Pines services and all service locations. The role involves attending various meetings, assessing client needs, providing linkage and follow-up for medical and social services, and advocating for clients.

Requirements

  • High School Diploma or approved equivalent combination of education and experience.
  • Valid driver's license is required as travel is a function of this position.

Responsibilities

  • Attend Integrated Treatment Plan meetings for clients enrolled in Care Coordination.
  • Attend Rule 79 Case Consultation Meetings to facilitate client needs.
  • Attend Thursday Integrated Consultation Meetings.
  • Assess clients' need for Primary Care (Medical), Dental Care, and Eye Care.
  • Provide needed linkage and follow-up to linkage.
  • Connect with clients to address identified needs on the Social Determinants of Health (SDoH).
  • Provide resources and referrals that would meet needs.
  • Close the loop on resources and referrals to ensure that clients have received the resources and/or engaged in the referred services.
  • Connect with clients to discuss services recommended by Mental Health Professionals following a Comprehensive Evaluation, Crisis Encounter, Psychiatric Evaluation, ITP meeting, or closing of services.
  • Submit referrals agreed upon by the client.
  • Advocate for clients to help ensure their needs are met.
  • Assess client satisfaction with services.
  • Provide MNSURE Navigation to individuals without insurance.
  • Provide Crisis Resources to clients and assist in connecting to Crisis Services.
  • Provide Health and Wellness Education and Information to clients.
  • Schedule and attend on-going visits for clients enrolled in the Care Coordination Program.
  • Work collaboratively with Community Partners – may include travel and working from community sites.
  • Manage daily reports in Electronic Health Record and workflow folders.
  • Maintain accurate and timely documentation of services provided to clients.
  • Provide outreach to clients regarding alerts received through EAS regarding recent Admit/Discharge from Hospitalization, Child/Teen Wellness.
