Care Coordinator - Behavioral Health

About The Position

The Care Coordinator - Behavioral Health provides direct outreach and connects consumers and clients in assigned programs with community resources. The role guides clients, consumers, and their families through the care continuum, establishing contact upon referral and maintaining contact throughout care and through self-management and/or active treatment.

Requirements

A bachelor’s degree in Social Work or a related field is preferred.
Two years of experience working with behavioral health consumers and families is preferred.
Experience in a team-based setting and collaborating with multiple providers is preferred.
Knowledge about community resources is preferred.
