About The Position

The Care Coordinator supports children and families enrolled in Pillars Community Health’s HealthChoice Pathways to Success program, an Illinois initiative providing comprehensive care coordination for children with complex behavioral health challenges. The position is scheduled Monday through Friday, 9:00 a.m. to 5:30 p.m., with flexibility to work evenings as client needs determine. It involves two to three days of in-person work at a Pillars Community Health office or in the community. Mileage for authorized work-related purposes is reimbursable. Compensation is $20.00–$25.00 per hour, based on experience, qualifications, bilingual skills, and internal comparisons.
