About The Position

The Care Coordinator serves as a link among the providers and community relationships involved in a client’s care, helping ensure continuity across physical health, behavioral health, and social services. The schedule is Monday–Friday, 9:00 a.m.–5:30 p.m., with flexibility to work evenings as client needs determine. Pay is $20–$25 per hour, based on experience, qualifications, bilingual skills, and comparison with similar employees.
