About The Position

Clarvida is seeking a Behavioral Health Care Coordinator to support children, adolescents, and families through community-based behavioral health services. The role provides case management support, coordinates care, and advocates for clients by connecting them with resources and services that promote stability, growth, and improved outcomes. It is a hands-on, client-facing position requiring strong communication, flexibility, and collaboration with treatment teams in community-based settings, including in-home and community locations in Jacksboro and Knoxville, Tennessee.

Requirements

• Bachelor’s degree in a Human Services discipline from an accredited college or university.
• Experience working with children and/or adolescents in a therapeutic, community-based setting.
• CPR and First Aid certification, or willingness to obtain certification before working with clients.
• Strong organizational, communication, and teamwork skills.
• Ability to work flexible schedules and travel between service locations.

Responsibilities

• Assist in the case management of children and adolescents.
• Support clients and families in achieving treatment and service goals.
• Collaborate with multidisciplinary treatment teams to develop and evaluate care plans.
• Advocate for individuals and families and connect them with resources.
• Coordinate services and referrals to community organizations.
• Maintain accurate, timely, and compliant documentation.
• Work flexible hours and across multiple locations as required by program needs.
