The Care Coordinator-Specialty role supports primary care physicians by managing proactive care, conducting outreach, and utilizing technology for patient management. The Care Coordinator will report to the Practice Manager.

Requirements

  • Medical Assistant certification or program completion.
  • Proficiency in computer applications including Excel, Word, Outlook, Nextgen, and PDF.
  • Excellent internal and external customer service skills.
  • Ability to maintain the highest degree of confidentiality.
  • Tact in dealing effectively with patients, providers, and employees while maintaining confidentiality.
  • Ability to work independently, handle high volume, and manage multiple tasks.
  • Reliable attendance.
  • Highly organized and detail-oriented.

Responsibilities

  • Receive internal referrals from primary care providers for specialty services within CSV.
  • Process prior authorizations required for insurer approval of consultations.
  • Review charts to assess the appropriateness, urgency, and medical necessity of specialty consultations.
  • Schedule consultation appointments based on appropriateness, urgency, and medical necessity.
  • Utilize standing orders from specialty physicians to gather necessary laboratory data or imaging studies before visits.
  • Develop and implement a system for processing and tracking outgoing referrals ordered by specialty physicians, ensuring clients follow up and recovering reports.
  • Conduct pre-visit planning for specialty consultations and follow-up encounters, obtaining medical records when appropriate.
  • Monitor new patients entering provider panels and analyze provider capacity to accept new patients.
  • Manage patient transfers within and outside CSV for assigned Primary Care Physicians (PCPs).
  • Facilitate PCP change requests initiated by patients, obtaining approval from current and future PCPs and updating the practice management system.
  • Facilitate PCP change requests initiated by providers, obtaining agreement from the new PCP.
  • Facilitate communication with outside providers for patients transferring care outside of CSV to ensure continuity.
  • Proactively engage priority patients to promote clinic accessibility and reduce unnecessary Emergency Room utilization.
