Bilingual Spanish Chronic Disease Management Health Coach (Remote)

Vesta Healthcare is seeking a passionate and dedicated Health Coach to join their team. The ideal candidate will work closely with members and caregivers to support them in managing their chronic conditions and acute exacerbations. They will empower members to become active participants in their own health outcomes and assist them in overcoming barriers to better health. The Health Coach should have experience in disease and chronic care management, as well as a Health Coaching Certification or a Bachelor's degree in Health and Wellness. Fluency in Spanish is also required.

• Work with individuals to help them improve their health and manage their chronic conditions
• Follow up with members to provide ongoing support and ensure their success in meeting their health goals
• Empower members to become active participants in their own health outcomes
• Assist members in overcoming barriers to better health
• Utilize biometric values and motivational interviewing techniques to collaborate with members to drive improvements in clinical and health outcomes
• Speak with members about disease and chronic care management
• Obtain Health Coaching Certification or have a Bachelor's in Health and Wellness to help people improve their overall health and manage their chronic conditions
• Have at least 1 year of recent health coaching experience
• Be fluent in Spanish in addition to English
• Have motivational interviewing training/experience
• Have a customer-centric focus with the ability to build relationships and gain trust quickly
• Have proven administrative abilities, with strong computer and software application skills
• Have an obsession with helping others improve their health
• Have the ability to work independently and be organized, tech-savvy, and highly motivated

• Experience speaking with members about disease and chronic care management (required)
• Health Coaching Certification or Bachelors in Health and Wellness to help people improve their overall health and manage their chronic conditions (required)
• At least 1 year of recent health coaching experience (required)
