About The Position

Join Wake County as our Bilingual Housing - Social Worker and help support individuals while building stronger communities! The Housing Social Worker provides outreach and engagement, intensive case management and group support to guests experiencing homelessness at the 234-bed South Wilmington Street Center (SWSC). This position is part of a multidisciplinary team delivering substance abuse services, mental health support, job coaching and job skills training, veteran services and case management to support guests in transitioning to stable housing. This role conducts assessments to obtain relevant social history and identify guest needs and connects individuals to appropriate social, medical, legal, financial, employment, behavioral health and housing resources.

Requirements

  • Bachelor's degree in Social Work or related field
  • One year of job-related experience
  • Valid Driver's License and "safe" driving record
  • Equivalent education and experience are accepted
  • Solid verbal and written communication skills and active listening ability
  • Spanish language fluency
  • Ability to work independently while adapting to change and managing multiple priorities
  • Proficient in Microsoft Office (Excel, Word) and data entry
  • Ability to build collaborative working relationships to accomplish goals
  • Knowledge of effective procedures for collecting, reviewing and monitoring information
  • Sound judgment, problem-solving and decision-making skills
  • Ability to train and support clients toward self sufficiency
  • Ability to meet deadlines, manage tasks and deliver timely results

Nice To Haves

  • Working knowledge of substance abuse, mental health and trauma-informed care practices
  • Ability to assess needs and connect guests to appropriate community resources
  • Familiarity with local service providers and referral processes
  • Over two years of experience working with At-Risk Populations
  • Fluent Spanish language skills

Responsibilities

  • Provide ongoing case management services and support guests in developing individualized service plans focused on achieving housing stability and self-sufficiency
  • Maintain accurate, complete and timely documentation of all guest interactions and services provided
  • Monitor guest progress and compliance with Center policies and program expectations
  • Facilitate or support group sessions and programming for guests
  • Collaborate with internal staff and community partners to coordinate services and support successful housing outcomes
