Bilingual Social Worker
About The Position
DaVita is hiring a Full-time Bilingual Social Worker to join their team. This role is crucial in supporting patients and their families through the difficult transition to life on dialysis. The Social Worker will help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease. The position offers the opportunity to build meaningful relationships in an intimate outpatient setting, be part of a supportive team, and receive performance-based rewards. Requirements
- Must be Bilingual in Spanish.
- Meets all state required regulations to practice in the dialysis setting.
- Master's degree in Social Work (MSW) required with a specialization in clinical practice.
- Demonstrated knowledge of government and private insurance programs.
- Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days.

- Support and advocate for patients transitioning to life on dialysis.
- Help patients understand their rights and responsibilities.
- Guide patients in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.
- Build meaningful and long-term relationships with patients and their families.
- Commit to improving patient health through clinical goal-setting and quality improvement initiatives.
