VillageCare is looking for a self-motivated and passionate Social Worker for this Full Time position working from the comfort of home. This role will provide telephonic assessments of members' status and needs. Requirements

Master's Degree
Current NYS LMSW license and registration
Minimum of three (3) years job-related experience in MLTC preferred
MUST LIVE NY/NJ/CT

Nice To Haves

Bilingual - Spanish, Cantonese, Mandarin, Russian, and/or Ukrainian preferred

Responsibilities

Provide all aspects of care management such as planning, coordination, assessment of health, and psychosocial status of the members.
Identify health, environmental, and psychosocial risks for assigned members and recommend appropriate interventions.
On-going assessment of the effectiveness of the Member's plan of care.
Assist interdisciplinary team with crisis intervention as needed.
Assist with community resource identification, including entitlements such as food stamps, housing applications, HEAP/SCRIE etc.
Identify issues/concerns of Social Determinates of Health.
Connect members to community resources that can assist them.
