About The Position

Kitsap Mental Health Services (KMHS) is seeking a Bilingual Care Coordinator to support children, youth, and families enrolled in our WISe program. This role helps families navigate services, build supportive teams, and work toward goals that matter most to them. Services are delivered in homes, schools, and community settings where support can be most effective. Requirements

  • Bilingual: Spanish and English Fluency required
  • Bachelor’s degree in psychology, social services, or behavioral health field.
  • Agency Affiliated Counselor Registration.
  • Must be at least 21 years old.
  • Must have been licensed for a minimum of three years.
  • Must successfully complete a motor vehicle history check.
  • Must possess and maintain a current, valid driver’s license in the state of Washington.
  • Must have reliable, insured transportation.

Nice To Haves

  • Minimum 2 years of job-related experience
  • Experience in co-occurring disorders treatment, individual and group therapy, vocational services, or substance abuse treatment
  • The capacity to work well with children and families.

Responsibilities

  • Provide care coordination services for assigned children, youth, and families enrolled in WISe.
  • Deliver direct services including family support, crisis intervention, care coordination, and case management.
  • Facilitate Child & Family Team (CFT) meetings that promote family voice, choice, and participation throughout the wraparound process.
  • Coordinate participation in related meetings including IEP conferences, court proceedings, Family Team Decision Making meetings, and other family support meetings.
  • Identify natural supports and encourage their involvement in the family's treatment and support network.
  • Develop and maintain Plans of Care that reflect family goals, strengths, needs, and action steps.
  • Ensure Plans of Care are updated following each Child & Family Team meeting.
  • Complete assigned action steps and coordinate follow-through among team members.
  • Build relationships with community partners and identify services that support family needs and goals.
  • Assist families in accessing housing, education, healthcare, transportation, social services, and other community resources.
  • Complete Child and Adolescent Needs and Strengths (CANS) assessments and reassessments according to program requirements.
  • Use CANS results to guide treatment planning and service coordination.
  • Develop, implement, evaluate, and revise treatment goals in collaboration with the treatment team.
  • Educate and support families while advocating for client rights, preferences, and culturally responsive services.
  • Coordinate care with DSHS, schools, courts, housing providers, medical providers, and other community partners.
  • Support clients in accessing medical, dental, psychiatric, and behavioral health services.
  • Provide crisis intervention and coordinate with Designated Crisis Responders and crisis services when appropriate.
  • Document services and client progress in accordance with agency, contractual, and regulatory requirements.
  • Participate in staff meetings, treatment planning reviews, and team consultations.
  • Provide education regarding mental health, substance use, and overall wellness.
  • Monitor clients with Less Restrictive Orders (LROs/LRAs) and coordinate extensions or related court processes when needed.
