About The Position

The Center for Health Equity & Community Wellness (CHECW) aims to eliminate health inequities that lead to premature mortality. CHECW works to increase the visibility of socially unjust policies and advocates for redress for the most impacted NYC communities. They address inequities across community and healthcare systems in partnership with various organizations, focusing on social determinants of health. CHECW is composed of several bureaus, including the Bureau of Bronx Neighborhood Health, which works with clinical and community partners to expand access to high-quality, culturally responsive programs for chronic disease prevention and management, particularly for residents facing significant health challenges. The Bureau of Bronx Neighborhood Health seeks to hire two (2) Asthma Community Engagement Coordinators to establish and maintain effective relationships with key local organizations and community partners.

Requirements

  • A baccalaureate degree from an accredited college and two years of experience in community work or community centered activities in an area related to the duties described above; or
  • High school graduation or equivalent and six years of experience in community work or community centered activities in an area related to the duties as described above; or
  • Education and/or experience which is equivalent to '1' or '2' above.
  • All candidates must have at least one year of experience as described in '1' above.

Responsibilities

  • Establish and maintain effective relationships with key local organizations and community partners to identify and address the health needs of underserved communities.
  • Manage special projects for the Bronx Asthma Program, including the development, implementation, and evaluation of asthma promotion projects in the Bronx.
  • Identify and assess local partnerships and provide technical assistance for development and/or partnership enhancement.
  • Coordinate activities for partnership building and improving community-level service coordination.
  • Support the development of Bronx Asthma Program outreach materials, including coordinating with the CHECW communications team and providing input on materials.
  • Provide services to the community and maintain ongoing communication with community partners, local leaders, and groups to explain Bronx Asthma Program interventions and initiatives.
  • Coordinate activities for partnerships with other agency programs.
  • Attend and deliver presentations and updates at community meetings and events.
  • Supervise and train health promotion staff and interns to ensure effective program delivery.
  • Occasionally attend community events during the evening and on weekends.
