About The Position

The Associate Center Clinical Director directly supervises and trains primary care providers in an assigned center and is accountable for execution of the core care model, including clinical quality, efficiency, outcomes, and clinician and patient satisfaction. The role has both direct patient-care and leadership responsibilities. The amount of time devoted to each depends on factors such as provider capacity, market needs, center size, patient membership, and direction from the Market Clinical Director. In the patient-care portion of the role, the Associate Center Clinical Director independently cares for older adult patients with acute and chronic illnesses and takes accountability for their care and outcomes. The position includes assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic needs. The director leads the care team for patients who can come to the office and coordinates with transitional-care teams and other resources for patients who cannot. Leadership duties include mentoring and coaching providers, supporting center operations and clinical leadership, and helping improve center performance and membership. The position may involve covering for other providers and participating in patient and provider recruiting. Local travel may be required, as well as initial national training and occasional regional or national travel for recruiting or training.
