About The Position

The Associate Center Clinical Director directly supervises and trains primary care providers (PCPs) at an assigned center and is accountable for execution of the core care model, including clinical quality, efficiency, outcomes, and clinician and patient satisfaction. The role is accountable for the center’s clinical outcomes and combines direct patient care as a PCP and/or other clinical duties with leadership responsibilities. The division of time depends on factors such as PCP capacity, market needs, center size, patient membership, and direction from the Market Clinical Director. The position provides care for older adult patients with acute and chronic illnesses, taking responsibility for their care and outcomes while consulting specialists when needed. It coordinates care across settings, including hospitals, skilled nursing facilities, long-term care, and patients’ homes. The director leads the center care team and weekly clinical meetings, mentors and coaches PCPs, supports center operations and clinical leadership, and contributes to recruitment and center membership growth. Local travel and occasional regional or national travel for training or recruiting may be needed. The role requires flexibility to work evenings, weekends, and/or holidays as needed.
