About The Position
The Assisted Living Care Coordinator provides care and support to residents in a cluster care setting, rather than one-on-one private duty. The role works closely with an interdisciplinary team to assist residents with activities of daily living, light housekeeping and meal support, escorting within the facility, and social engagement and companionship. The position is in person at The Osborn, a not-for-profit senior living community in Rye, New York.
Requirements
- Active New York State Home Health Aide (HHA) Certificate.
- Be compassionate, dependable, and team-oriented.
- Be able to commute to Rye, NY 10580.

Responsibilities
- Assist residents with activities of daily living, including bathing, dressing, grooming, and toileting.
- Provide light housekeeping and meal support.
- Escort residents within the facility.
- Provide social engagement and companionship.
- Work closely with the interdisciplinary team to support residents.
