About The Position

At Country Meadows, nursing leadership is about more than overseeing care—it’s about supporting residents in a homelike environment while guiding and developing the teams who care for them every day. We are seeking an Assistant Director of Nursing (ADON) LPN who brings strong clinical skills, a hands-on leadership style, and a commitment to both resident well-being and team support. In this role, you will work closely with the Director of Nursing and campus leadership to provide day-to-day clinical oversight, support staff development, and help ensure consistent, high-quality care in a setting where relationships matter and leadership is present.

Requirements

  • Current, unencumbered RN or LPN license in the state of employment
  • 2–4 years of clinical nursing experience in assisted living, personal care, or long-term care
  • Strong knowledge of resident care practices, care planning, and team support
  • Experience supervising or guiding care staff, including coaching and training
  • Proficiency with electronic health records (e.g., PointClickCare) and basic computer systems
  • Strong communication, organization, and problem-solving skills
  • CPR certification required

Nice To Haves

  • Prior supervisory or leadership experience in a healthcare setting preferred

Responsibilities

  • Provide direct nursing services as needed to support resident health and safety
  • Conduct and review head-to-toe assessments for new and returning residents
  • Participate in wellness team meetings and coordinate ongoing resident assessments
  • Monitor quality of care delivery and ensure consistency in clinical practices
  • Complete and review DHS-required assessments for assigned residents
  • Ensure care and support plans are communicated and followed by care teams
  • Maintain open communication with residents, families, physicians, and care teams
  • Support resident wellness through ongoing evaluation and coordination of care
  • Provide hands-on supervision of personal care staff and support nursing team oversight
  • Assist in recruitment, hiring, onboarding, and training of care staff
  • Participate in coaching, feedback, and performance evaluations
  • Support staffing coordination in collaboration with the Director of Nursing
  • Foster a positive, team-oriented environment focused on collaboration and accountability
  • Support compliance with company policies, procedures, and regulatory requirements
  • Assist with infection control practices, tracking, and reporting
  • Ensure medication associate training and monitoring meets required standards
  • Monitor for safety concerns, including potential abuse or neglect, and respond appropriately
  • Maintain accurate documentation and serve as a PointClickCare resource for staff
  • Collaborate with dining, life enrichment, therapy, and chaplain services to support resident well-being
  • Provide social, emotional, and supportive care to residents and families
  • Partner with interdisciplinary teams to enhance resident experience and outcomes
