About The Position

Palo Verde Behavioral Health Hospital is seeking an experienced Intake Assessment Counselor/Coordinator. The role is the initial contact for patients and referral sources and provides clinical assessment and determination of ongoing clinical needs. The Counselor conducts telephonic risk assessments and in-person clinical assessments, then determines the appropriate level of care in consultation with the hospital psychiatrist. For admitted patients, the Counselor facilitates the admission process and provides clinical information for ongoing treatment. The role also conducts clinical evaluations and admissions for the Intensive Outpatient Program (IOP). Palo Verde Behavioral Health Hospital provides behavioral healthcare for children, adolescents, adults, and senior adults experiencing mental health and addictive illnesses. Services include partial and intensive outpatient hospitalization and specialized chemical dependency treatment programs.

Requirements

Master’s degree in a related field.
Three to five years of experience is preferred.
LMSW, LPC, LISAC, or RN license is preferred.

Responsibilities

Serve as the initial contact for patients and referral sources.

Facilitate the admission process for patients admitted to the hospital.
