Aging Life Care Manager – The Villages, FL

Diversified Life Care Partners•The Villages, FL

About The Position

Diversified Life Care Partners is seeking experienced healthcare and social services professionals to build a career in Aging Life Care Management. This autonomous, relationship-based role helps older adults and their families navigate complex care needs. The Aging Life Care Manager assesses the whole person and situation, solves problems, coordinates care, advocates for clients, and helps families make informed decisions. This is not bedside nursing, traditional therapy, home health, or facility case management. The position serves The Villages, Florida, and surrounding areas, including Leesburg, Wildwood, and Lady Lake. Previous Aging Life Care Management experience is not required; the company will train the selected professional in its approach to comprehensive assessment, care planning, advocacy, documentation, family communication, and ongoing care management. Day-to-day work may include visiting clients at home, assessing medication-related difficulties, accompanying clients to physician appointments, discussing increasing care needs with family members, coordinating services with home-care agencies, and evaluating whether assisted living communities can meet clients’ needs. The manager follows situations over time, coordinates the people and services involved, and helps ensure that care plans work in practice.
