About The Position

This role supports older adults and eligible individuals by providing information, assessments, referrals, and direct assistance across Agency programs. It requires clear communication, attention to detail, and teamwork across various Agency units. As an Aging Case Aide 1, you will perform the following duties: Initial Contact: Serve as the first point of communication for individuals seeking Agency services Assessment Work: Conduct interviews, complete intake forms, explain Agency programs, and enter information into Agency databases Service Coordination: Refer consumers to Care Management staff and coordinate services following Agency procedures Caseload Maintenance: Manage initial and annual reassessments for the In-Home Meals Program Daily Living Support: Assist participants with personal care, mobility, and safe transfers Vital Monitoring: Obtain and record vital signs and assist with rehabilitative activities as directed Meal Support: Assist with meal preparation, serving, feeding, special diets, and clean-up Activity Assistance: Assist in recreational and therapeutic activities and supervise participants during program activities

Requirements

  • One year of public contact work, which involved obtaining or providing information; or Any equivalent of experience and training.
  • Meet the PA residency requirement.
  • Be able to perform essential job functions.
  • Pass a background investigation.
  • Pass a drug screening.
  • Satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
