An Aging Care Manager 3 plays a vital role in helping older adults live safer and more stable lives. Requirements

  • One year as an Aging Care Manager 2 or Aging Program Assessor; or One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.
  • Meet the PA residency requirement.
  • Be able to perform essential job functions.
  • Pass a background investigation.
  • Submit and approve satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.

Responsibilities

  • Provide care management services that support older adults and improve their quality of life
  • Assist older adults who face abuse, neglect, exploitation, or abandonment
  • Serve as a liaison to social service agencies and community resources to meet client needs
  • Consult with staff in other program areas regarding assessments, diagnoses, and treatment plans
  • Develop plans that promote safe living situations or improve daily living conditions
  • Perform supervisory responsibilities and participate in subordinate work as needed
