About The Position

Wayne County Area Agency on Aging is seeking a compassionate, organized individual to help older adults access services and supports that allow them to remain active, safe, and independent in the community of their choice. This role supports older adults and eligible individuals by completing assessments, arranging services, and providing clear information about available programs. The work helps consumers access resources, understand options, and maintain their independence in a safe and informed way.

Requirements

Six months as an Aging Care Manager 1 or a County Caseworker 1; or Successful completion of the County Social Casework Intern program; or Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training.
Meet the PA residency requirement.
Able to perform essential job functions.
Pass a background investigation.
Conditional offer of employment will require a drug screening.
Must submit and approve satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.

Responsibilities

Collect consumer information, document details, and explain available programs and eligibility
Arrange services for consumers based on simple or complex needs in cooperation with community resources
Conduct evaluations in homes, facilities, or other settings to determine service needs and access to resources
Maintain caseloads, complete reassessments, and gather financial and personal information when required
Prepare letters, reports, and records while responding to inquiries from families and professionals
Track daily activities, update databases, and maintain confidential information with accuracy
