About The Position

This position focuses on developing care plans and managing services for older adults. Work primarily supports consumers for the OPTIONS program which provides assistance to Pennsylvania residents age 60 and older who would like to stay in their own homes. As an Aging Care Manager 2, you will perform the following duties: OPTIONS Caseload: Visit consumers in their homes for intake, general casework, and assessments that involve interviewing, evaluating needs, and developing a plan of care for services Service Coordination: Arrange and monitor services for consumers with simple to complex needs, help consumers complete applications and utilize resources Assessment Work: Conduct initial, six-month, and annual reassessments by phone and in person at homes, facilities, and hospitals Record Management: Document case history, care plans, and ongoing activity; complete reports; and maintain accurate files Community Collaboration: Work with families, providers, and community agencies to support consumer needs Teamwork Support: Serve as backup for the Person-Centered Care Manager and receive one PCC referral at a time.

Requirements

  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or Successful completion of the County Social Casework Intern program; or Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training.
  • Meet the PA residency requirement.
  • Be able to perform essential job functions.
  • Submission of criminal history reports will be required for a conditional offer of employment.
