About The Position

Westmoreland County Area Agency on Aging is seeking a professional full-time Aging Care Manager II to help them make a positive impact on the families and community. Their mission is to assist and enable older and disabled persons to live their lives as independently as their circumstances will allow and in the best possible and desired manner. This position focuses on supporting vulnerable older adults by managing a caseload and addressing complex situations.

Requirements

• Six months as an Aging Care Manager 1 or a County Caseworker 1; or Successful completion of the County Social Casework Intern program; or Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training.
• Meet the PA residency requirement.
• Able to perform essential job functions.

Responsibilities

• Maintain a caseload of vulnerable consumers and use independent judgment to handle routine and complex cases
• Collect details from consumers, families, neighbors, and others to identify social, economic, emotional, health, or physical concerns
• Investigate Reports of Need involving potential abuse, neglect, abandonment, or financial exploitation
• Review gathered information to identify risks and apply regulations to determine substantiation
• Provide protective interventions such as court petitions, emergency relocation, or referrals for community services
