Requirements

  • One year as an Aging Care Manager 2 or Aging Program Assessor; or One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.

  • Be able to perform essential job functions.
  • Pass a background investigation.
  • A conditional offer of employment will require a drug screening.
  • Must pass PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.

Responsibilities

  • Conduct and oversee complex investigations into alleged abuse, neglect, exploitation, or abandonment.
  • Assess the medical, social, psychological, financial, and environmental needs of people in high-risk and complex circumstances.
  • Develop, implement, and monitor service plans to reduce or eliminate risk and assist older adults with accessing needed resources.
  • Recommend and coordinate advanced protective services and referrals with internal and external organizations, care providers, and community organizations.
  • Recommend and coordinate court interventions and testify in legal proceedings.
  • Perform supervisory duties, provide guidance, review casework for quality and compliance, participate in training, and assist with other levels of care management when needed.
