About The Position

As an Aging Care Manager 2 with the Northumberland County Area Agency on Aging (AAA), you help older adults overcome challenges and stay safe in their communities. This position supports individuals and families by linking them to programs that make daily life easier. You make a positive impact through careful planning and thoughtful guidance. Your work strengthens the well-being of those who rely on compassionate care. This role centers on assessing needs and coordinating services that enhance the quality of life for older adults.

Requirements

• Six months as an Aging Care Manager 1 or a County Caseworker 1; or Successful completion of the County Social Casework Intern program; or Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training.
• Meet the PA residency requirement.
• Be able to perform essential job functions.
• Pass a background investigation.

Responsibilities

• Evaluate social contacts, daily living activities, cognitive functioning, physical health, and available resources
• Connect older adults with agencies, hospitals, clinics, courts, and community organizations that support their needs
• Conduct detailed assessments of medical, social, psychological, and housing conditions to support effective care planning
• Work with professional staff and community partners to discuss diagnoses and plans of treatment
• Assist older adults in managing income, accessing Social Security and Medicare, and understanding legislation that affects benefits
• Prepare social studies, case histories, reports, and records
