The Aging Care Manager 2 position with the Pike County Area Agency on Aging (AAA) offers a chance to make a real difference in the lives of older adults. Requirements

  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or Successful completion of the County Social Casework Intern program; or Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training.
  • Meet the PA residency requirement.
  • Be able to perform essential job functions.
  • Pass a background investigation.
  • Pass a drug screening (conditional offer of employment).
  • Submit criminal history reports (conditional offer of employment).

Responsibilities

  • Develop informed care plans based on thorough medical, social, psychological, and functional assessments
  • Ensure legal and regulatory time frames are met while following all policies, procedures, and statutes
  • Conduct detailed investigations into situations involving possible abuse, exploitation, abandonment, or self neglect
  • Complete referrals, arrange needed services, and monitor follow up to maintain service quality
  • Perform periodic reviews to identify changes in consumer status and adjust support plans accordingly
  • Provide guidance to staff and the public on legal, medical, and privacy matters affecting protective service consumers
