About The Position

Westmoreland County Area Agency on Aging is seeking a professional full-time Aging Care Manager 2 to help make a positive impact on the families and communities they serve. The mission is to help older and disabled persons live their lives as independently as their circumstances will allow and in the best possible and desired manner. This role focuses on protecting adults aged 60 and older who may be at risk of abuse, neglect, exploitation, or unsafe living conditions. The position involves identifying and coordinating services to support consumers in meeting their social, economic, emotional, health, and physical safety needs.

Requirements

• Six months as an Aging Care Manager 1 or a County Caseworker 1; or Successful completion of the County Social Casework Intern program; or Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training.
• Meet the PA residency requirement.
• Be able to perform essential job functions.

Responsibilities

• Maintain a caseload of vulnerable consumers, visit homes, and follow up with each consumer and their formal and informal supports as needed.
• Advocate for consumers to help connect each person with the services they need to be safe, stable, and connected to their communities; testify at court proceedings.
• Interview consumers, family members, and other contacts; review relevant records; and evaluate information and apply regulations to determine appropriate next steps.
• File emergency petitions, provide referrals for community based services and assessments, and pursue other remedies as warranted by investigative findings.
• Work with legal, medical, financial, and other appropriate professionals to reduce hazards, end danger, and stabilize consumers.
• Maintain case records including intakes, assessments, care plans, correspondence, referrals, and progress notes in accordance with agency policy and directives.
