About The Position

The Schuylkill County Office of Senior Services is seeking patient, compassionate, and organized candidates to join their team as Aging Care Manager 2s. The position is responsible for evaluating the needs of older adults, developing plans, and coordinating services to provide clients with the most effective possible care and support. Training and team support are provided. This role is rewarding for those who enjoy helping older adults find the services they need to stay active and safe while remaining in their chosen community setting.

Requirements

  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or Successful completion of the County Social Casework Intern program; or Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training.
  • Meet the PA residency requirement.
  • Be able to perform essential job functions.
  • Pass a background investigation.
  • Pass a medical examination and a drug screening (conditional offer of employment).
  • Submit and approve satisfactory criminal history reports, including PA State Police and FBI clearance (conditional offer of employment, Older Adult Protective Services Act).
  • Submit and approve satisfactory criminal history reports, including PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance (conditional offer of employment, Child Protective Services Law).

Responsibilities

  • Conduct comprehensive assessments to evaluate each person’s level of need for facility, agency, or referral services; problems related to independent living; and social, emotional, and other capabilities.
  • Include all available assessment results and information to recommend a comprehensive plan that helps maintain the consumer’s independence for as long as feasible.
  • Meet with individuals, their families, and hospital officials at their home, a hospital, or other care facility; explain agency services; and help older adults and families complete forms and connect with services.
  • Make referrals to appropriate community services, coordinate service delivery, and provide on-going assessment, re-evaluation, and follow-up.
  • Complete all paperwork and enter confidential client information into state databases within required deadlines.
  • Regularly coordinate services with medical teams, community agencies, financial institutions, care homes, and other resources; participate in outreach efforts; and respond to client emergencies and protective services.
