About The Position

This trainee position participates in formal and on-the-job training in the methods, procedures, policies, and regulations governing our agency’s work with vulnerable consumers. The role begins by accompanying and observing more senior Aging Care Managers on visits to consumers and as they complete required assessments and documentation. Successful candidates gain proficiency, fully learn the position, and are able to become increasingly independent in completing assignments. As an Aging Care Manager 1, you will learn and perform the following duties: Knowledge Development: Study and apply policies, procedures, and regulations for the Caregiver Support Program (CSP), the OPTIONS Program, care management, and assessment Assessment Assistance: Conduct home visits with consumers, caregivers, and family members; help conduct interviews to determine consumer eligibility; complete and record assessments on a laptop computer Care Planning: Create care plans with consumers and family members during face-to-face meetings, over the phone, and in written correspondence; coordinate services; and conduct follow-up Consumer Support: Assist with benefit applications, accessing community resources, and working with outside providers; calculate reimbursements and cost sharing Case Documentation: Maintain complete and accurate case history, forms, and other records in agency systems

Requirements

  • Three years as an Aging Case Aide 2
  • A bachelor's degree which includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services, or a closely related field
  • An equivalent combination of experience and training
  • Meet the PA residency requirement
  • Able to perform essential job functions
