Requirements

  • Three years of experience in public or private social work and a Bachelor's Degree; or Any equivalent combination of experience and training including successful completion of 12 semester hours of college level courses in sociology, social welfare, psychology, gerontology, or other related social sciences.
  • Meet the PA residency requirement.
  • Must be able to perform essential job functions.
  • Conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
  • Conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.

Responsibilities

  • Supervise Aging Care Managers who provide care management to older adults
  • Determine client eligibility for programs and make appropriate referrals
  • Prepare monthly, quarterly, and annual reports on department status
  • Monitor services under contract such as home support, personal care, and legal services
  • Maintain contact with community agencies and exchange information on services for the elderly
