About The Position

A Path of Care Preferred Pathways of Hugo, Oklahoma, is hiring a full-time ADvantage Case Manager serving Hugo and the surrounding area. The Case Manager provides in-home case management to help members live safely and independently in their homes. The position includes assessing members’ needs, developing care plans, monitoring members monthly, initiating preventive interventions, and coordinating services. The Case Manager also participates in in-service programs and ADvantage training. The schedule is flexible and self-scheduled, with approximately 50% of work performed remotely and 50% involving travel and in-home member assessments and monitoring.

Requirements

• Oklahoma RN or LPN license.
• Bachelor’s degree.
• One year of paid professional experience working with an aging or disabled population.
• CPR certification within 60 days of hire for RN/LPN candidates only.
• Valid driver’s license and good driving record.
• Reliable transportation and proof of insurance.
• Ability to travel long distances.

Nice To Haves

• Experience with the ADvantage Program is preferred.

Responsibilities

• Complete initial assessments of members’ needs.
• Develop plans of care.
• Conduct monthly monitoring of members.
