About The Position

The Advanced Practice Provider (APP) provides comprehensive primary care services to adult patients through the University of Virginia Health System’s innovative Mobile Care Unit and associated population health programs. This position supports UVA Health’s mission to improve access to high-quality care and advance health equity through innovative, community-based care delivery models. The APP practices in collaboration with an interdisciplinary care team and contributes to Interactive Home Monitoring (IHM) and primary care bridging programs, supporting continuity of care for patients across the care continuum. The position is administratively affiliated with the Department of Population Health and professionally supported by the Center for Advanced Practice, UVA Health’s professional home for advanced practice providers. Hours are Monday-Friday with an expectation of 32 patient-facing clinical hours weekly. A cover letter included in your application packet is preferred.

Requirements

  • Graduate of an accredited graduate school of nursing or physician assistant program with a master’s degree.
  • Department and unit specific experience.
  • License or license eligible as an Advance Practice Provider (Nurse Practitioner, Physician Associate, Certified Registered Nurse Anesthetist, and Certified Nurse Midwife) as set forth by the state of Virginia.
  • If Nurse Practitioner, license to Practice as a Registered Nurse in the Commonwealth of Virginia is required.
  • Must demonstrate and maintain all credentialing and licensing requirements of UVA Health System, the Virginia Board of Medicine or the Virginia Board of Nursing, and appropriate certifying bodies.
  • Current Basic Life Support (BLS) required.
  • Current PALS/NRP, as required, specific to position.
  • Current Advanced Cardiac Life Support (ACLS) as required, specific to position.

Nice To Haves

  • 3+ years of experience in adult primary care, community health, or mobile health settings
  • Experience caring for underserved or high-risk patient populations.
  • Familiarity with population health, care management, or transitional care models.
  • Experience with telehealth, remote patient monitoring, or virtual care platforms.
  • Strong interpersonal, communication, and teamwork skills.
  • Experience practicing within an academic medical center or integrated health system.
