About The Position

Under direction, performs work in providing social work services on cases involving complex social problems and social treatment plans; performs responsible and difficult casework, directs service programs or provides work direction to paraprofessional staff; and performs related work as assigned. This role involves providing information, referrals, and case management services to individuals, conducting comprehensive initial and on-going assessment interviews to evaluate individuals’ physical, economic, educational, and mental background, and developing case plans with recommendations for clients in assigned program areas. The position also requires gathering and compiling evidence for court appeals, maintaining regular client contact, implementing solutions, monitoring client progress, and maintaining detailed documentation of client participation and case records. Additionally, the role involves determining the presence of major mental illness, drafting internal procedures, and evaluating the impact of regulatory changes on programs.

Requirements

  • A bachelor's degree from an accredited four-year college or university with a major in social work, psychology, sociology, or closely related field Or A bachelor's degree from an accredited four-year college or university with a major in any field and one year of experience as a social worker in a public or private social services agency.
  • Knowledge of the principles and techniques of interviewing and recording in social work and the ability to apply them.

  • Valid MN driver’s license.
  • Experience working with Microsoft Office (Word, Excel, PowerPoint, Outlook, etc.).

Responsibilities

  • Provide information, referrals, and case management services to individuals.
  • Conducts comprehensive initial and on-going assessment interviews to evaluate individuals’ physical, economic, educational, and mental background; develops case plans and makes recommendation for clients in assigned program areas.
  • Gathers and compiles evidence and data required to complete documentation for court appeals and dispositional review hearings.
  • Conducts regular assessments and maintains regular client contact via phone, home visits, office visits, or site visits with other agencies involved in the case plan.
  • Implements solutions and interventions which includes monitoring and evaluating clients’ progress in meeting goals and expectations of the case plan; prepares and maintains progress reports.
  • Maintains detailed documentation of clients’ participation in case records, on-going case narratives; compiles data for various reports, summaries, and memos; completes mandated State and local forms and adheres to program deadlines.
  • Determine if there is documentation indicating the presence of major mental illness.
  • Drafts internal procedures for assigned programs.
  • Evaluates the impact of changes in regulation on programs.
  • Provide information and referral services to ensure compliance with regulations.
  • Evaluate an individual’s need for Level II screening.
  • Receive referrals for Preadmission Screening workers.
  • Determine need for and refer for diagnostic assessment by a mental health professional.
  • Review completed assessments and determine if client is or is not in need of active treatment, and recommend an appropriate course of treatment and care.
  • Coordinate services based on eligibility.
  • Performs other duties of a comparable level/type, as required.
  • Coordinate with other positions within the department for client services.
  • Attend continuing education and training opportunities.
  • Conduct Adult Protection Multidisciplinary Team meetings.
  • Receive and evaluate referrals through Social Service Information System (SSIS) or a variety of other sources.
  • Contact client to assess needs and desire for mental health services.
  • Complete (SSIS) application for case management.
  • Make referrals to appropriate agencies for services.
  • Monitor case and develop an Individual Family Community Support Plan (IFCSP) to monitor goals.
  • Document services by maintaining a SSIS case file and completing eligibility reviews.
  • Maintain appropriate case records and documentation.
  • Complete social and medical histories for commitments and collaborate with the County Attorney.
  • Coordinate agency programs and services with Mental Health Initiative Components.
  • Participate in the development of new and innovative programs and services.
  • Participate in case staffing, planning activities, and case consultation.
  • Receive vulnerable adult reports, screen and assess cases, open cases as necessary, arrange for and monitor services.
  • Prepare for and testify in court as requested by the County Attorney.
  • Diagnoses and determines the need for chemical dependency treatment services.
  • Coordinates all recovery services, including intake, assessment, coordination of care, treatment planning, family coordination, client crisis situations, testify in court when needed, case documentation, prepetition screening process for any civil commitment.
  • Provides case management services and responds to requests for civil commitment due to chemical dependency issues.
  • Coordinates services when needed with probation, our legal system, treatment providers, and the family service agency.
  • Generates payments to service providers of clients on disability waivers.
  • Each case has multiple service agreements that need to be monitored and updated to meet clients changing needs.
  • Assists the family in establishing a guardianship by working with an attorney, providing documentation, and care coordination.
  • Conducts phone discussions with individuals having inquiries into social services or needing assistance.
  • Conducts face-to-face interviews/visits with family and individuals to determine level of risk, level of intervention and recommends the appropriate services.
  • Evaluates individual needs and provides information and/or referrals when appropriate.
  • Completes application for services when deemed necessary.
  • Enters intake in SSIS.
  • Completes necessary case opening paperwork as determined by Agency policy when services have been requested.
  • Complete the functional assessment (FA) by assessing with the child and family receiving CMH for strengths, resources, supports, needs, functioning, health problems and conditions, safety, vulnerability, and injury risk.
  • Develop an ICSP and IFCSP with the client and include the following: Goals and the specific services, activities for accomplishing each goal, schedule for each activity, and frequency of face-to-face contact with client.
  • Determine achievement of the goals and objectives in the IFCSP.
  • Complete voluntary placement agreements and Out-of-Home placement sheets.
  • Ensure child MA and IV-E requirements are met.
  • Make appropriate use of STAY funds to teach independent living skills in conjunction with placement.
  • Completes child welfare checks when necessary and when screened out for child protection.
  • Notifies law enforcement within 24 hours and jointly investigates reports that may involve criminal offenses.
  • Completes courtesy child protection reports for other counties.
  • Notifies court and the County Attorney and completes paperwork for substantiated abuse and Child in Need of Help or Prevention (CHIPS).
  • Initiates and supervises Foster Care Placement.
  • Provides case management for children in foster care and completes Out-of-Home Placement plans.
  • Provides transportation to visitation.
  • Facilitates communication between foster parents and natural parents.
  • Attends all Individual Education Plan meetings.
  • Reviews Out-of-Home Placement Plans (OHPP) every 90 days.
  • Makes referrals to Support for Emancipation and Living Functionally (SELF) to teach independent living skills in conjunction with foster parents.
  • Contacts foster parents regarding foster child’s progress.
  • Arranges visitations with parents and relatives.
  • Maintains SSIS and case record documentation including Difficulty of Care forms as required and ensures transportation to court.
  • Provides training for foster parents.
  • Applies for appropriate funding for Out-of-Home placements within 7 days of placement.
  • Keeps financial workers, schools and other appropriate agencies informed of any changes in placement through written notice.
