Access Services is hiring a Young Adult Mental Health Case Manager for their Transition to Independence Process (TIP) program, serving young adults aged 16-26 across Bucks County. The TIP program utilizes an evidence-based, strength-based, person-centered, and youth-driven model to support young adults in achieving their future goals across five key domains: Education; Employment & Career; Living Situation; Community Life Functioning; and Personal Effectiveness & Wellbeing. The role involves empowering young adults, providing in-the-moment coaching, and facilitating problem-solving. This is not traditional case management, but a supportive approach to mental health. The TIP program is expanding to meet the growing mental health needs of transition-age young adults. The team offers a supportive environment, smaller caseloads for more meaningful engagement, and comprehensive training in the TIP Model by a Certified Site-Based Trainer. The position is full-time and requires on-site presence, with no remote or hybrid options. Travel within the community is necessary, utilizing a personal or agency vehicle.
