About The Position

Orange County Alter Behavioral Health is an adult mental health treatment provider with a network of upscale inpatient residential facilities in Orange County and San Diego. As a Care Path Manager (Case Manager), your primary focus is managing a caseload of clients and supporting their progress throughout treatment. You will serve as a consistent point of contact, helping ensure each client receives individualized, high-quality care in a structured residential setting. You play a critical role in shaping both the client experience and the performance of the team supporting them.

Requirements

  • Experience in mental health case management or residential treatment

  • Comfortable making decisions in a fast-paced, dynamic environment
  • Knowledgeable in residential mental health operations and regulatory standards
  • Committed to maintaining a safe, ethical, and client-centered environment
  • High School Diploma required
  • At least one (1) year of experience in behavioral health case management required
  • Valid California Driver’s License required
  • CPR & First Aid certification required (or ability to obtain prior to start)
  • Must pass Live Scan fingerprint clearance
  • Must be legally authorized to work in the United States

Nice To Haves

  • CADAC-1 or equivalent certification
  • Knowledge of trauma-informed care practices
  • Experience working with multidisciplinary teams
  • Familiarity with EMR systems
  • Bachelor’s or Master’s Degree preferred
  • At least one (1) year of leadership or supervisory experience preferred

Responsibilities

  • Manage a caseload of approximately 6 clients in a residential setting
  • Serve as the primary point of contact for assigned clients throughout their treatment stay
  • Monitor progress toward treatment goals and communicate updates with the clinical team
  • Facilitate check-ins, support daily structure, and reinforce therapeutic programming
  • Advocate for client needs during treatment planning and interdisciplinary meetings
  • Support care coordination, including admissions, transitions, and discharge planning
  • Represent client needs in treatment planning and interdisciplinary discussions
  • Support continuity of care and coordination across services
  • Promote adherence to program structure, expectations, and care plans
  • Respond promptly and effectively to behavioral or emotional escalations
  • Utilize de-escalation techniques and involve clinical leadership when appropriate
  • Ensure a safe, structured, and therapeutic residential environment
  • Support Care Coordinators in daily responsibilities
  • Provide real-time feedback, performance guidance, and mentorship
  • Assist with staff development
  • Oversee daily residential operations to ensure efficiency and consistency
  • Maintain accurate documentation and support audit readiness
  • Ensure compliance with DHCS, DSS, and Joint Commission standards
  • Facilitate groups and support admissions and discharges
  • Coordinate communication between departments to ensure continuity of care
  • Support implementation of policies, procedures, and best practices
