Teaches independent living skills to consumers and assists with personal care as needed. This role involves planning and conducting daily classes/activities, instructing clients on their Individual Program Plan (IPP) objectives, and adhering to program guidelines including Medicaid and DDS standards. The position also requires providing habilitation training, completing required in-service and out-of-area training, and maintaining daily documentation on various data sheets and progress notes. Responsibilities include supervising contract work, organizing supplies, providing active treatment, and ensuring proper lifting techniques. The role demands a professional attitude and may involve personal care needs of clients. A significant aspect of the job includes serving as a driver for participant day trips, which may be required daily, and demonstrating proficiency in operating motor vehicles, including passenger vans. Personal vehicle use may also be required, necessitating adequate liability coverage. This is a safety-sensitive position that requires maintaining confidentiality in compliance with Friendship Community Care Non-Disclosure Policy and HIPAA Policies.
