Teaches independent living skills to consumers and assists with personal care as needed. This role involves planning and conducting daily classes/activities, instructing clients on their Individual Program Plan (IPP) objectives, and adhering to program guidelines including Medicaid and DDS standards. The position also requires providing habilitation training, completing required in-service and out-of-area training, and potentially attending meetings after hours. Responsibilities include following the Policy and Procedures Manual, transporting clients, attending Annual Reviews, maintaining daily Medicaid notes and DOL information, and keeping classrooms and training areas clean. Documentation on Data Sheets, Progress Notes, AD Goal Sheets, and Young Homes daily log is essential, as are Behavior, Seizure, and Incident Reports. The role involves supervising contract work and the quality of products made by clients, assisting with class supplies, and providing Active Treatment Requirements for Young Homes clients. Compliance with proper lifting techniques and hover/sling procedures is necessary, along with maintaining a professional attitude. Personal care needs of clients must be met, and the employee will serve as a driver for participant day trips, potentially requiring daily driving and the use of a personal vehicle with adequate liability coverage.
