About The Position

The Manager of Admissions, Transfers & Discharge is responsible for leading, directing and overseeing the admissions, transfer, and discharge processes for individuals and families seeking services across the organization’s waivered programs. This positions exercises discretion and judgement in coordinating complex, multi-stakeholder processes, evaluating operational readiness, and making recommendations that significantly impact services delivery, resource allocation and regulatory compliance throughout each stage of the process for the individuals in which we serve.

Requirements

  • High school diploma or equivalent required.
  • Minimum of 3–5 years of experience in healthcare, human services, or IDD services required.
  • Valid Georgia driver’s license required and insurance for travel throughout Metro Atlanta.

  • Associates or Bachelor's degree preferred
  • Proven experience coordinating complex, multi-stakeholder processes preferred.
  • Project management training or certification (e.g., PMP or equivalent coursework) preferred.

Responsibilities

  • Lead and manage all admissions, transfers, and discharge processes from initial inquiry through completion using structured project management approaches.
  • Develop, maintain, and continuously improve workflows, timelines, and communication processes to ensure efficiency and consistency.
  • Serve as the primary point of contact for all stakeholders throughout each process.
  • Identify and coordinate with external stakeholders, including individuals, families, support coordinators, third-party landlords, healthcare providers, and regulatory agencies (e.g., Georgia Department of Behavioral Health and Developmental Disabilities).
  • Ensure all communications are timely, clear, and professional, and that required information is shared and received at each stage of the process.
  • Partner with internal departments (Residential, Day Services, People & Culture, Finance, Facilities, Safety, Fleet, etc.) to ensure alignment and readiness.
  • Lead the Clinical Evaluation Team in reviewing medical and clinical documentation, coordinating assessments, and determining service appropriateness.
  • Facilitate site visits and assessments in partnership with clinical and operational teams.
  • Provide informed recommendations to Executive and Senior Leadership regarding admissions, transfers, and discharges, including considerations related to clinical needs, safety, staffing, financial impact, and compatibility within residential settings.
  • Coordinate and manage all required internal and external approvals prior to admissions, transfers, or discharges.
  • Partner with external stakeholders (e.g., DBHDD, support coordinators, families) to complete required documentation, site visits, and approvals.
  • Ensure all processes meet regulatory, contractual, and organizational requirements.
  • Collaborate with Facilities, Fleet, and other departments to ensure environments are prepared prior to admission or transfer.
  • Coordinate necessary modifications, adaptive equipment, or technology to support individual needs.
  • Oversee all logistical arrangements related to residential moves, ensuring a smooth transition experience.
  • Oversee the accuracy, completeness, and timely submission of all documentation related to admissions, transfers, and discharges.
  • Ensure documentation is properly shared with stakeholders and uploaded into appropriate systems.
  • Supervise the Support Associate responsible for documentation and administrative coordination.
  • Conduct post-process reviews (after-action analyses) following admissions, transfers, and discharges.
  • Gather feedback from stakeholders and identify opportunities for improvement.
  • Provide recommendations to leadership to enhance processes, efficiency, and stakeholder experience.
