About The Position

Norwood Crossing is seeking a Clinical/Hospital Liaison to provide clinical support, education, and advocacy that helps patients, families, and healthcare partners achieve smooth care transitions and optimal outcomes. The role involves assessing patients’ needs, explaining health conditions and treatment options, and helping coordinate individualized care. The position offers a commission structure intended to reward performance and provides access to earned wages before payday through RAIN.

Requirements

• At least 2 years of experience in a skilled nursing setting, with an understanding of post-acute care needs.
• Strong communication and interpersonal skills for engaging with diverse individuals and professional teams.
• Ability to work independently with initiative and sound judgment, and to contribute effectively to a collaborative team.

Nice To Haves

• Current RN or LPN license is preferred but not required; preference is given to candidates with an RN license.

Responsibilities

• Conduct comprehensive patient assessments to understand individual needs.
• Provide clear, empathetic education about health conditions and treatment options.
• Collaborate with healthcare providers to develop and implement individualized, holistic patient care plans.
• Communicate compassionately with patients and families, addressing their healthcare needs, concerns, and questions.
• Participate in case conferences and team meetings to discuss patient progress, share insights, and plan for optimal outcomes.
• Act as a liaison among patients, families, and healthcare providers to support smooth transitions and coordinated care.
