About The Position

Liberation Programs is seeking an Admissions Coordinator for its Stamford Outpatient Services. This role is responsible for ensuring the smooth and efficient admission of individuals seeking treatment for substance use disorders. The coordinator will conduct assessments, determine the appropriate level of care, manage waiting lists, coordinate referrals, and ensure programs operate at full capacity. The position also involves engaging referrals, providing orientations to treatment, and maintaining accurate data on admissions and utilization.

Requirements

  • Minimum of two years' experience in behavioral health screening, assessment, and referral processes.
  • Familiarity with data management and electronic medical records systems.
  • Strong organizational skills and ability to handle multiple priorities and meet deadlines while providing exceptional customer service.
  • Excellent computer skills.

Nice To Haves

  • CT State License preferred (LCSW, LMFT, LADC, LPC)
  • Knowledge of state and federal behavioral health regulations preferred.

Responsibilities

  • Coordinate and manage admissions processes, including managing walk-ins and calls, handling referrals, screening applicants, scheduling intake dates, prioritizing admissions, and managing waiting lists.
  • Assist applicants not admitted with referrals to alternate settings and document reasons for non-admission.
  • Manage recoveree admissions to create a welcoming environment, ensuring completion of financial information and pre-authorization.
  • Complete initial assessments and determine the appropriate level of care.
  • Identify and review crisis issues with treatment teams.
  • Coordinate with physicians and medical assistants for timely medical screenings.
  • Coordinate counselor assignment with the Program Director.
  • Follow up with recoverees within 10 days of admission to ensure completion of orientation to treatment.
  • Network with and conduct outreach to referral resources, representing Liberation Programs at community meetings and establishing collaborative relationships with partner services and agencies.
  • Report on meetings attended, partnership development opportunities, and referrals generated from community meetings.
  • Manage census and utilization data, reporting daily admissions, bed vacancies, and waiting list status to ensure programs are at full capacity.
  • Provide monthly reports on admissions outcomes data, including the number of applicants, screened, admitted, and the average time to complete the admissions process.
  • Report on discharges and identify trends regarding utilization rates.
  • Assist with special projects as needed under the direction of the CEO or CPO.
