•$90,000 - $100,000•Remote

About The Position

Aledade is seeking an Account Manager to serve as a strategic partner, business coach, and advocate for medical practices in the Greater Philadelphia area. The Account Manager is the day-to-day anchor for assigned practice relationships, using data to identify performance gaps and working with physicians and healthcare executives to improve financial results and patient outcomes through value-based care (VBC) contracts. The role owns account relationships across financial performance, quality and clinical management, population health, care management training, HEDIS and Stars performance, and operational improvement. It uses analytics to monitor practice performance, develops project plans, supports training, and communicates contract goals and performance results to practice leaders and executives. The position is primarily remote, with approximately 20–25% regional travel for on-site partner visits—typically once per month per account for one to three days, often as day trips. Candidates must live within reasonable driving distance or have convenient regional flight access. Aledade PBC is a public benefit corporation founded in 2014 that supports independent primary care practices, health centers, and clinics in delivering better care and succeeding in value-based care. The organization describes its culture as collaborative, inclusive, and remote-first, with a mission to strengthen primary care and improve health care for patients, practices, and society.
