About The Position
Pathways Hospice is a newly formed company dedicated to providing a continuum of care and long-term care services to patients in the Greater Houston Area. We are committed to delivering excellent patient experiences and are looking for dedicated associates to join our team. This role focuses on providing high-quality healthcare and customer service.
Requirements
- Valid RN license in the State of Texas.
- Hospice experience (at least 1 year).
- Knowledge of home health regulations and general healthcare compliance.
- Excellent communication and people skills are essential.
- Ability to work independently and as part of a team.
Nice To Haves
- Experience with Home Care Home Base (HCHB) EMR (preferred but not required).
Responsibilities
- Oversee nursing care team.
- Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
- Plan, organize, and direct home care services.
- Conduct assessments of patient care needs and develop care plans.
- Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
- Prepare timely, complete, and accurate clinical notes.
