About The Position

Hospice of the East Bay (HEB) is a community-based nonprofit hospice and palliative care organization that has been serving patients and families with compassion and dignity for nearly 50 years. We provide hospice and palliative care in patients' homes and in our six-bed inpatient hospice facility, ensuring every patient receives exceptional care wherever they are most comfortable. Our interdisciplinary team works together to support not only our patients but also their loved ones, offering bereavement services for a minimum of one year after a loss. Through our sponsored care program, we remain committed to providing care regardless of a patient's ability to pay. At Hospice of the East Bay, our employees are at the heart of everything we do. This position serves patients in Vallejo, Hercules, Richmond, Emeryville, Oakland, and surrounding areas.

Requirements

  • Current California RN license
  • Minimum of one year of acute care nursing experience
  • CPR certification
  • Valid California driver’s license and reliable transportation
  • Proficient in basic computer applications and electronic health records

Nice To Haves

  • Experience in hospice or home health
  • Certification in Hospice and Palliative Care Nursing
  • IV therapy experience

Responsibilities

  • Recognize and respond to signs of active dying and patient transition
  • Effectively assess and manage pain and symptoms based on individualized care plans
  • Educate patients, families, and caregivers using clear and compassionate communication
  • Document all assessments, interventions, and patient progress accurately and promptly
  • Deliver holistic, patient- and family-centered care across diverse care settings
  • Honor patients' cultural and spiritual beliefs throughout their journey
  • Collaborate with interdisciplinary team members and external providers to ensure quality care
  • Coordinate care with attention to insurance, authorizations, and cost considerations
  • Facilitate care transitions such as General Inpatient (GIP) or Respite when needed
  • Supervise LVNs and Home Health Aides with clarity and support
  • Participate in team meetings, training sessions, and quality improvement activities
  • Maintain compliance with all applicable state, federal, and accreditation standards
