DaVita Kidney Care is expanding its services by bringing industry-leading clinical quality to patients on-site at skilled nursing facilities (SNF) through a new team called DaVita Skilled Nursing Dialysis Services (DSD). This initiative aims to reduce the burden on patients who are transported for dialysis and lower healthcare costs. DSD Services teammates will leverage DaVita's resources to develop and grow this new service line. The ideal candidate thrives in a start-up environment, is passionate about improving patient lives, and can work autonomously while collaborating with cross-functional teams. In this role, you will be responsible for the complete ongoing care of each patient to ensure their dialysis treatment stays on track, working in conjunction with dialysis RNs. The primary work schedule is Full-Time, based at a site dialysis den within Northridge Health and Rehabilitation, with potential support for additional surrounding sites. Operations run Monday through Saturday, with scheduled days varying based on market needs. This role requires multitasking and a high level of autonomy and critical thinking to provide comprehensive patient care in an independent setting. You will constantly monitor patients before, during, and after dialysis treatment, including measuring and recording stats, patient observations, and hemodialysis machine setup.
