About The Position

The Home Care Transition Nurse is responsible for assessing, planning, and developing referrals. This role involves establishing and maintaining professional relationships with key physician groups and referral sources, acting as a liaison to referral sources on behalf of Catholic Home Care, identifying potential market growth opportunities and program expansion, and serving as a customer service representative in a professional and supportive manner at all times.

Requirements

  • Registered Nurse with current licensure in NYS
  • Customer Service and Marketing skills required

Nice To Haves

  • 2 or more years of home care experience preferred
