About The Position

Halcyon Home is hiring a Home Health LVN PRN - Per Visit to support our growing geriatric home health population in Waco, TX and surrounding areas. We offer competitive pay, and wonderful team support. This role is for an LVN with a heart for seniors and a passion for making a real difference in the lives of the patients they serve. The ideal candidate is a charismatic communicator who has the gift of reading people, loves engaging with people, and takes pride in getting everyone taken care of. A dedicated LVN with home health experience (or experience in a related field and a desire to learn about home healthcare) who is a team player and thrives in a supportive environment is encouraged to apply.

Requirements

  • Graduate of an accredited school of vocational/practical nursing.
  • Active, state license as a licensed vocational/practical nurse, or a multi-state license issued by a Nurse Licensure
  • One year of experience as a licensed vocational/practical nurse in a clinical care setting required.
  • Nursing skills as defined as generally accepted standards of practice.
  • Good interpersonal skills.
  • Proof of current CPR certification.
  • Reliable transportation and valid auto liability insurance.

Nice To Haves

  • Home health experience, preferred
  • Homecare Homebase platform experience is a PLUS!

Responsibilities

  • Under the direction of the RN, assists in identifying the patient’s physical, psychosocial, and environmental needs as evidenced by documentation.
  • Participates in planning and implementing care in partnership with the patient, representative (if any), caregiver(s), and the RN. Provides services that are ordered by the physician as indicated in the plan of care.
  • Provides patient and caregiver education.
  • Provides care using infection control measures that protect both staff and patient.
  • Assures the continuity of care through delivery of quality patient care.
  • Provides effective communication to the patient, family, team members, and other healthcare professionals as evidenced by clinical notes, case conferences, and communication notes. Completes appropriate documentation in a timely manner.
  • Monitors assigned cases to ensure compliance with requirements of third-party payor.
  • Participates in the Agency’s QAPI program.
  • Participates in the Agency sponsored in-service trainings.
  • Demonstrates commitment, professional growth, and competency.
  • Promotes the Agency philosophy and administrative policies to ensure quality of care.
