About The Position

Willow Creek Health Care Center is seeking an experienced and compassionate Licensed Vocational Nurse (LVN) – Admissions Nurse to join our team on a weekends-only schedule. The Admissions Nurse plays an important role in welcoming new residents to our facility and ensuring a smooth transition into skilled nursing care. This position works closely with nursing leadership, admissions, physicians, residents, families, and the interdisciplinary team to ensure each new admission is completed accurately, efficiently, and with excellent customer service.

Requirements

  • Current and active California LVN license in good standing.
  • Current CPR/BLS certification as required.
  • Strong clinical assessment and documentation skills.
  • Knowledge of medication reconciliation and physician orders.
  • Excellent communication, organization, and time-management skills.
  • Ability to manage multiple admissions and priorities in a fast-paced healthcare environment.
  • Strong attention to detail and commitment to providing excellent resident care.

Nice To Haves

  • Previous skilled nursing, long-term care, post-acute, or rehabilitation experience preferred.
  • Previous admissions or charge nurse experience is a plus.
  • Experience with electronic health records preferred.
  • PointClickCare experience is a plus.

Responsibilities

  • Complete nursing assessments for newly admitted residents.
  • Review hospital records, discharge summaries, medication lists, physician orders, and other clinical documentation prior to and upon admission.
  • Verify that admission orders are complete and accurately entered into the resident's medical record.
  • Complete medication reconciliation and communicate discrepancies or concerns to the appropriate physician or nursing supervisor.
  • Ensure residents are appropriately assessed upon arrival and that immediate clinical needs are addressed.
  • Coordinate with the Admissions Department and nursing leadership regarding anticipated admissions.
  • Communicate with hospitals, physicians, pharmacies, families, and other healthcare providers as necessary.
  • Welcome residents and families and provide education regarding the admission process and facility services.
  • Ensure required admission documentation is completed accurately and within required timeframes.
  • Initiate resident care plans and appropriate clinical documentation.
  • Identify resident risks, including falls, skin concerns, nutritional needs, pain, behaviors, and other clinical issues.
  • Notify the Director of Nursing or appropriate supervisor of significant findings or changes in condition.
  • Coordinate with dietary, therapy, social services, MDS, and other departments regarding new resident needs.
  • Maintain accurate and timely documentation in the electronic health record.
  • Follow infection prevention, safety, HIPAA, and facility policies and procedures.
  • Maintain confidentiality of resident and facility information.
  • Provide professional and compassionate communication with residents and their families.
  • Assist the nursing team with resident care needs as appropriate.
  • Perform additional nursing and admissions-related duties as assigned.
