About The Position

Sage Group HealthCare Staffing is seeking Home Health Registered Nurses for 13-week travel/contract positions in Everett, WA. This role involves providing and coordinating care for assigned patients in their homes, developing and implementing nursing care plans, supporting patient recovery, and empowering patients and families through education on disease management. The nurse will also be responsible for case management, providing clinical oversight, and ensuring adherence to nursing practice standards and facility policies. Key duties include assessing patients, documenting changes in status, notifying physicians of significant changes, and completing all patient visit charting within 24 hours. The territory to be covered is Snohomish County.

Requirements

  • Registered Nurse: 2 years - Required
  • Recent/Current Home Health Nursing experience: 1 year within the last 3 years - Required
  • OASIS experience - Required
  • Home Care Home Base (HCHB) charting - Required
  • Must have valid Driver’s License and Personal Auto Insurance
  • ASN or an equivalent – Required
  • RN License for State of Washington or Compact if applicable - Required
  • BLS Certification from AHA or American Red Cross - Required

Nice To Haves

  • BSN or an equivalent – Highly preferred

Responsibilities

  • Provide and coordinate care for assigned patients in comfort of their homes.
  • Develop and implement nursing care plans for all types of visits including Start of Care and admissions as assigned.
  • Support patient recovery and ability to stay at home.
  • Empower patients and families to reach healthcare goals by educating them on disease management.
  • Case management and provide clinical oversight to interdisciplinary teams.
  • Provide nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
  • Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
  • Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
  • Notify physicians of significant changes in status.
  • Document and complete charting for all patient visits within 24 hours.
