About The Position

HealWell, LLC is seeking a motivated and compassionate APRN/PA to join our growing wound care team. This position provides specialized wound care to patients across a variety of settings, including private residences, skilled nursing facilities, long-term care facilities, and outpatient clinic settings. Our providers work independently while collaborating with patients, families, facility staff, and other members of the healthcare team to develop individualized treatment plans focused on wound healing, limb preservation, and improving overall quality of life.

Requirements

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program.
  • Current, unrestricted APRN/PA license with prescriptive authority in the State of Oklahoma.
  • Current national board certification.
  • Valid driver's license and ability to travel within the assigned service area.
  • Strong clinical assessment and decision-making skills.
  • Ability to work independently and manage a mobile patient schedule.
  • Excellent communication, organization, and documentation skills.
  • Comfortable providing care to medically complex patients in home, facility, and outpatient settings.

Nice To Haves

  • Two or more years of experience as an advanced practice provider.
  • Previous wound care experience.
  • Wound care certification.
  • Providers who are not currently certified may be required to obtain an approved wound care certification within six months of hire.
  • Clinically confident, dependable, organized, and comfortable practicing with a high degree of autonomy.
  • Takes pride in excellent patient care.
  • Communicates well with the interdisciplinary team.
  • Committed to developing advanced wound care knowledge and skills.
  • Experience with complex wounds, pressure injuries, diabetic ulcers, arterial and venous ulcers, surgical wounds, debridement, compression therapy, negative pressure wound therapy, and other advanced wound modalities.

Responsibilities

  • Perform comprehensive patient and wound assessments.
  • Develop and manage individualized wound care treatment plans.
  • Accurately assess, measure, stage, and document wounds and wound characteristics.
  • Perform wound care procedures within scope of practice, including selective and excisional debridement as appropriate.
  • Utilize advanced wound care products, therapies, and treatment modalities.
  • Assess vascular status and perform or interpret appropriate vascular screening, including ABI/TBI when indicated.
  • Identify barriers to wound healing and coordinate appropriate referrals, diagnostic testing, and follow-up care.
  • Collaborate with physicians, nursing staff, home health agencies, facility staff, patients, families, and other members of the healthcare team.
  • Educate patients, caregivers, and facility staff regarding wound care, pressure injury prevention, nutrition, offloading, compression, and other factors affecting wound healing.
  • Maintain accurate, thorough, and timely clinical documentation.
  • Practice in accordance with HealWell policies, HIPAA requirements, applicable state and federal regulations, and professional standards of practice.
  • Maintain company-issued equipment and wound care supplies.
  • Travel to assigned patient locations as required.
