About The Position

This role is focused on promoting preventive care and wellness services across the patient population while supporting primary care practices in achieving quality outcomes, enhancing patient engagement and satisfaction, and improving population health management. The Wellness RN collaborates closely with providers and interdisciplinary care teams to identify patients with high-risk needs, develop individualized action plans, and connect patients with appropriate resources and services.

Requirements

  • Associate Degree Nursing (ASN) or higher required.
  • ASN with 6 years clinical experience or BSN with 4 years clinical experience.
  • Current license to practice as Registered Nurse in the State of Maryland, or if resident of a state that participates in Nurse Multi-State Compact agreement, holds Registered Nurse License from that state that is designated as "Compact" or "Multi-State Privilege" using primary source verification and provides additional documentation as required by the Maryland Board of Nursing. RN License from a compact state with a designation of "Single State" does not meet the requirement and nurse must obtain a Maryland license.
  • Licensed to practice as a Registered Nurse in the State of Maryland.
  • Current BLS (CPR) required in compliance with policy.
  • Ability to apply the standards of the professional nursing practice with compassion and empathy.
  • Basic computer skills required.
  • Must have high level of organizational and communication skills.

Nice To Haves

  • Bachelor's of Science (BSN) in Nursing or higher preferred.
  • Experience in managed care environment preferred.

Responsibilities

  • Providing preventive and wellness-focused nursing care and education.
  • Supporting value-based care initiatives, including programs such as Maryland Primary Care Program (MDPCP).
  • Promoting patient engagement in age-appropriate wellness activities and preventive screenings.
  • Assisting primary care practices in improving quality metrics and population health outcomes.
  • Educating patients on disease prevention, health maintenance, and appropriate healthcare utilization.
  • Collaborating with physicians, advanced practice providers, and care team members to coordinate care and address identified health risks.
  • Performing clinical duties in compliance with organizational policies, procedures, and regulatory requirements.
  • Participating in performance improvement and quality initiatives designed to advance patient outcomes and value-based care goals.
  • Completing other duties as assigned.
